It is a web-based application that includes screens for each of the 9^th SOW themes- Patient Safety, Prevention (EHR, CKD, and Disparities), Care Transitions and Beneficiary Protection. It allows QIOs to enter specific information for each theme as requested by the theme GTL. It also includes a Document Storage area to store narrative documents, and allows project officers to search and approve/reject specified documents. *PATRIOT is replacement of Program Activity Reporting Tool (PARTner). PARTner was used for the 8^th SOW to collect the program activity information requested by CMS.*
